Coordinating Access to Care

The ERE programs facilitate a smooth, coordinated transition from an acute episode in the hospital or emergency department to ongoing community resources.

The goal is to identify client needs and connect clients to appropriate supports in service of improving quality of life and reducing future, preventable hospital utilization.

Adult Emergency Room Enhancement (ERE)

Staff from six community mental health centers work with clients referred by Eastern Region hospitals to address behavioral health, physical health, and basic needs. To be eligible for services, clients must be uninsured or under-insured and reside in the Eastern Region. 

ERE Outreach Specialists offer the following services:

Meet with Clients

Identify/address needs such as housing, transportation, and food

Motivate them to engage in treatment services and/or community supports

Youth Emergency Room Enhancement (YERE)

Outreach Specialists from three community mental health centers serve youth ages 6-17 with significant behavioral health needs who are referred from Eastern Region hospitals, emergency departments, and youth clinics. To be eligible for services, youth must be uninsured or under-insured and reside in the Eastern Region.
